Media/Photo Release. I give Bloom Wellness Collective permission to use photographs of myself or the participant for any Bloom Wellness Collective promotional materials.

Medical Release

The staff of Bloom Wellness Collective will treat minor medical needs accordingly ie: band-aid on cut, ice pack on swelling. In the case there is a major medical emergency ie: seizure, 911 will be contacted immediately and the ambulance expense will be the responsibility of the parent or guardian of the participant.

Liability Disclaimer and Release

Bloom Wellness Collective and its staff take all reasonable care in ensuring that its programs are safe. However, I agree that the participant will be engaging in physical activities that may involve some risk of injury. I acknowledge I have received advice from the participant’s physician with respect to any past or present injury, illness, health problem or any other condition or medication that may affect the participant's involvement in the Bloom Wellness Collective program(s). I assume the above risks and accept responsibility for any injury sustained by the participant and discharge and hold harmless Bloom Wellness Collective and its staff and volunteers from any liability arising from any injury to the participant or other persons or property caused by my or the participant's involvement in the Bloom Wellness Collective program(s).
